disclaimer: this is just my reading and i might be wrong. but: drow is pulling away because he got frightened by himself and what he might do if he 'lets go' during sex. and astarion is like, 'oooh, kinky!' the hand movement reminds me a lot of what he does after tasting the player's blood for the first time. he's either about to touch his bloodied lip, or has just touched it and might smell or taste his own blood off his fingers. he's smiling because there's nothing quite like novelty to spark the interest of a bored, bitter immortal.
