I’m imagining then a living underwear for the wearer’s front as the wearer goes for a run (having the bulge bounce and move in the underwear’s mouth etc), and then be living underwear for the wearer’s back as the wearer does squats at the gym (having the underwear’s head being massaged and squeezed as the muscle ass clapped with thier face in the middle)
