Aerys, her father, didn't go mad until he was kidnapped and held prisoner. It's unclear whether or not he was tortured (physically), but he was never the same after that. In fact, before his kidnapping, he and Tywin were relatively close, but afterwards, he got very paranoid and also started lusting after Tywin's wife. The point is, it seems Targs can go crazy after traumatic events. It's not that they are always mad, just predisposed to make rash decisions and paranoia. And that then leads them to madness and eventual delusion. Some fans even think it's paranoid schizophrenia, based on how some of them react. Such as Baelor locking up all his sisters and daughters so they can't get pregnant and Maegor having a bunch of hidden passages built into the red keep then killing all the architects and builders after it was complete.
