Artbook Status Update (Free Art Books)

We've just received the final art book layout from our designer and we are pushing with the print soon. As promised, we will be sending free art books out to those who are in the $50 tier.
But since many of you have been here for a very long time and have pledged well over $50 in the past year, we've decided to give out free art books to everyone who has cumulatively pledged over $50. We'll even handle the shipping cost.
In order for this to go smoothly, we need your cooperation.
Because Patreon doesn't give us your private information (as it shouldn't) we need to collect data to those who want the free art book. Please fill in the form below!
EXTREMELY IMPORTANT -
The deadline for free art book sign up is SEPTEMBER 1ST 2016, your local time. We can't accept orders after this so please please please make sure to sign up before September 1st. There will be no exceptions.
