The scene with Carmella and the therapist is so well written. I think it sums up who she is entirely and that's why it shocks her so much. It's truth shining on her life of lies. Also - this therapist recommends Tony read "Crime and Punishment" while Melfi suggested "The Art of War".

I think other people mentioned it, but yeah, they're not actually cousins. Italian-Americans, among other cultures (like mine--Lebanese and my wife's--Filipino) call close friends of the family "uncle" or "auntie" even though they're not blood-related.
